I've done a cursory look through parts of this thread and just wanted to confirm that all I need to do is email United customer service (or use the Contact Us form on the website) and include:
- my flight number
- date
- the fact I'm claiming UK261 compensation
- description of why the flight was late/canceled - i.e., that it was a mechanical issue
Since I will undoubtedly use a travel cert, I also understand that I can say I'd accept a $1000 ETC in lieu of the GBP520 cash.
Is that really all there is to it? I've never done this before.

Yes, you can still file for additional compensation. Besides $1000 EC or cash, they also offer 50K miles instead. They do not negotiate. You can also claim additional expenses or meals.
