When you search, it will show you the refundability terms. Here is an example of the text:
Standard Room Free Night

Cancellation Policy
11:59pm Hotel Time 1 Day Before Arrival Or Pay 1 Night Fee/Credit Card Required

Deposit Policy
Credit Card Guarantee Required

From 5,000 Points/Night
The way to read that is: Cancelation before 11:59pm 1 day before arrival results in a return of your points. Cancelation after that, or no show, results in a charge to your credit card for the cash price of the first night, as a cancelation penalty.

I've never seen a points rate that included breakfast, unless it explicitly says breakfast is included. So assume you do not receive breakfast.

EDIT: For this question:
The total is more than the per night points figure multiplied by the number of nights so I presume that includes all taxes and fees?

The per night price is the lowest price of any of the nights on your reservation. Each night will have its own price. So if you multiply the per night price by the number of nights, and it is asking for more points, then one or more of the nights in your reservation has a higher rate.

i.e. if it says "From 20,000 points" and your 5 night stay requires 120,000 points, then one night might be at 40,000 points, or two at 30,000, etc.

I've never been charged taxes on an award stay but others who have more experience might have. There is a specific policy against charging Resort Fees:

Waiver of Resort, Destination, or Facility Fee on Free Night Awards. Resort, destination, or facility fees, where charged by the applicable hotel or resort, will be waived for Members redeeming a Free Night Award. This benefit is not available at any property for which the applicable Supplemental Terms indicate as much. This benefit applies only to nights for which the Member has redeemed a Free Night Award. This benefit does not apply to any Room Upgrade Award, Points + Cash Award, or any non-stay award (e.g., a Dining, Spa and More Award or a Future Meeting Credit (defined in Appendix D)). In some instances, a hotel or resort may charge other fees (such as a “service fee”) that are independent of any resort, destination, or facility fee charged by that hotel or resort. Any such fees will not be waived for Members pursuant to this benefit.
