Very interesting question, when I did the course at the BA Global Learning Academy they ran exactly that scenario (ex-JNB to LHR) with a medical emergency over central Africa and we were told the options were back to JNB, on to MAD or a small unknown airfield somewhere much closer (the wrong answer!), so I guess it really is pretty limited.
