Yes, they do read the notes, or figure out which airport is the closest... eventually. Misconnected bags are often processed overnight.

I had a bag where it was misconnected and was intended for EWR. Whoever saw the Connecticut delivery address put it on 2 more flights - EWR-IAD-BDL, and have BDL agent to arrange the delivery.
