"N/A" indicates no, you don't.

The notation to footnote 5 is the same in each chart. Where medallion mileage bonuses are available, footnote 5 says " Medallion Mileage Bonus is based on Base Miles earned: Diamond - 120%; Platinum - 80%; Gold - 60%; Silver - 40%"
