Did you use any credits to pay for flight? There can be issues in getting full mileage credit when partially paying for flights with existing credits. FYI, it's 5 miles/dollar for base fare price (excluding government taxes and fees), not total ticket cost as noted at link above. Also, if you fly on any partner marketed flights, mileage credit uses a distance-based formula, rather than fare based.
