You can pay in one sense, if the child is in Club or First they get lounge access. But usually BA won't allow extra guests beyond one, particularly during school holidays. The exception would be if the child is under 2 years old.
Yes, I've seen people with an extra child turned away. It does seem to depend who is on the entry, and how busy they are.
If there is not a big advantage to booking the paid lounge early, I'd try BA and see how you go.
Remember there is a lounge at each end of gate A, so you can try twice (though a 5 minute walk), if the first turns you down. Also I think a Gate B lounge.
Or take two children in with your wife, leave a few minutes later and bring the other child in, if old enough to be unsupervised for a while. Its a busy place, with no exit scan, so you can always say that your wife and children have left to shop.
