Thank you KI-NRT for the feedback! Snagged the last room ("Sakura Room") for our dates at Nishimura Honkan thankfully it seems to be in the main building and not the annex. Interesting that it is advertised as having an in room open air bath- pictures show that it must have been recently renovated. What ended up being the deciding factor is that I have heard Kinosaki Onsen is relatively tattoo friendly and my SO has a couple so it will be nice for him to actually use the public ones for a change.
The Sakura room does indeed appear to have been recently renovated with modern vanity and a (semi-) open air bath, as well as a slightly bigger room now.
Kinosaki Onsen - as far as I understand - is now completely tattoo-friendly.
Forgot to mention that Nishimuraya Honkan (and all ryokans in Kinosaki, for that matter) don't have Onsen in the room (en-suite baths are regular water), but the communal baths at the ryokan ARE onsen.
