The Sakura room does indeed appear to have been recently renovated with modern vanity and a (semi-) open air bath, as well as a slightly bigger room now.

Kinosaki Onsen - as far as I understand - is now completely tattoo-friendly.

Forgot to mention that Nishimuraya Honkan (and all ryokans in Kinosaki, for that matter) don't have Onsen in the room (en-suite baths are regular water), but the communal baths at the ryokan ARE onsen.
