Once got E+ (two seats) just by asking at the customer service desk (the general desk in the concourse, not the gate) in LAX before boarding for LIH. Was on my honeymoon but honestly can't remember if I mentioned that fact or not. This was mid-January 2017 and the flight went out at least a third empty.
