I have a medication that is only available in 30-day increments in China and a few other countries I frequent. About 5 years ago, I explained this to my doctor in the US, and he wrote up a new Rx for me at 3x the dosage. I realize this approach might not work in cases where the higher dosage doesn't make sense under any circumstances, but I've never had any issues getting that Rx filled locally.
This is something I do too. For a couple meds the understanding US Dr rewrote the prescription in this way.
However, the most important medicine is one self-injection I must do every 2 months.. and there's no way to rewrite it.. also the specialist was not on board with that idea.
