THE U.S. FEDERAL GOVERNMENT REQUIRES ALL CUSTOMERS TO PROVIDE CONTACT TRACING INFORMATION.

Customers (including U.S. passport holders) entering or returning to the U.S. must provide contact-tracing information for just one customer on your reservation before checking in for your flight, as it is required by the CDC.

For visitors, the information provided must be the destination address – the address where you will be staying in the U.S. during your visit. For anyone who lives in the U.S., it will be your home address.

A quick tip
Be sure to complete the form online before checking in and arriving at the airport, to expedite document verifications and boarding processes. It is best to complete the form while connected to Wi-Fi before arriving at the airport, in many instances, the internet available at the airport is not reliable and will delay your check-in process.

Always have JetBlue at your fingertips by downloading out free mobile app - where you can get flight status alerts, manage your trip, get a mobile boarding pass (where available), and more.

Thank you for choosing to fly with us. We look forward to welcoming you on board soon.

Yes it's the same email. So this form has to be filled out online before check in or not? Did they ever sent this in previous years? I am pretty sure I never gotten this email previous in the last few years of flying even during covid.
