Sometimes with IRROPS and rebooking you may end up with a different fare for purposes of PQPs even though there's no charge to you. In this case it may be that somehow the agent booked you into a $650+ ticket and you got those PQPs.
Sometimes UA figures this out, sometimes they don't. I think if you call just about the buy up it's unlikely to trigger a review of the entire ticket, but it could.
