Could be wrong but I understood MAD / JED as a surface segment which should make it valid for a DONEx. Also makes sense since IB doesn't fly to JED and the OP didn't indicate any connection getting there.

Are any segments giving you economy only instead of business? That can cause pricing issues in the tool. I think your best bet is to call AA RTW desk with this and have them price it, you have the required AA overwater segments.
My only other bit would be why RJ for LHR-xAMM-MAD and not QR LHR-xDOH-MAD? On RJ the MAD flight is an A320 with recliners and the AMM lounge is nothing special at all (for that matter the 787 from LHR is also nothing special at all). With QR you'd get widebodies on both flights and the Al Mourjan lounge in DOH.
