From a bar I frequent, I'm told they order those and pay for them like anything else. It's not like they are making crystal clear 1.5in x 1.5in cubes of ice in their icemaker. That said its kinda strange to upcharge for it, and even $1.
I'd ask for it neat and regular ice on the side if you want to avoid that.
