Dont bother. You still have to pay 2200 JPY. you sure you want to do it for maybe 3 times?
3 years is as long as Global Entry IIRC.

Yes. TTP website for entry form, then VJW for customs. Or you can do paper form for customs but you might need to stay in a queue...
