Based on my conversation with the supervisor at LAS, who hands out a special MM coin that looks like a casino chip, each station receives a list of MM crossovers for that day. It is up to the station to communicate with the flight crew and set up the celebration ceremony.

A passenger on my ORD-EWR flight this past weekend hit the milestone, and someone from ORD presented him a box of chocolate, taking the picture of the passenger with the chocolate, and the captain presented the MM coin. The FA also made a special mention during the welcome announcement - "We would like to extend a special welcome to MileagePlus members, including [name], a new million miler". Albeit, this whole thing was one flight too early, as he will cross over into MM on his next flight to DXB.
