Corporate city slicker knee-jerk axing of heads culture leads to big smoking holes that contain what was once an aircraft and its charge of contents if applied to aviation.
Having trained in one of the other safety critical industries and having studied some aviation safety in a (fruitless) attempt to improve things, I am happy as a passenger that aviation is done the aviation way. Other industries could learn a lot.
