I always felt this way and wondered how I'd do it, but was super pleasantly surprised on my VS 787 flight 2 months ago; wasn't spacious by any means, but definitely enough room (and a side table) to avoid dragging anything on the floor or even bumping the walls. The PJs definitely made a difference in comfort when it came to sleeping. If only they could all have the A380 sized lavs, though...
