Only you know what you bought to be honest. The printout certainly suggests that they did exactly that. It would be contrary to the rules of almost any airline, but we do know some mileage brokers exist, posturing as OTAs and will sometimes buy miles from people and "sell" them in the form of tickets to people wanting to buy a cheap flight. If you booked through a major OTA (e.g. Expedia, Lastminute, etc) at a standard fares, then it is almost certainly a mistake on MH's part though no idea why or how. If, on the other hand, you bought from an obscure OTA and in particular if the fare was much lower than the price others were proposing, this may well be what happened.

In terms of the return, did you have one single PNR for both outbound and return or were they different booking references?
