As an anecdote, I traveled overseas a few years back with my wife and small child (about 5 years old at the time) and they allowed all three of us access to the MIA Flagship Lounge without issue. I actually checked ahead of time on a prior trip and they said it would not be an issue, and sure enough it was not. I would wager it’s up to the agent’s discretion.
