I do see ANA (and JAL) selling tickets arriving at either airport with a connection (international) at the other airport.
There are limousine (big buses) that go between the airports on a schedule. You have to transport your luggage (they put it under the seating area) and the buses are clean, safe and on time. The airlines don't pay IIRC it's about USD 20 and takes about an hour.
