I got the leisure version of this with mostly Main/Comfort options and a couple of PS. "Would consider other airlines" for all and wrote some pretty scathing comments at the end.

Restoring change fees on Main for me would to lead to a pretty major drop in my spend w/Delta. I book a lot of tickets when I'm 70% sure of my travel plans and credit the ones I don't end up flying. A lot of times the feasibility of the trip isn't confirmed until quite short notice, at which point the fares would be much higher and I would just not take the trip. The rest is slightly less ugly, though it cheapens the brand - I pay more to fly Delta partially because it's a consistent experience. At what point does it become equal to just taking whatever ULCC happens to be cheapest? Even out of SLC, the majority of my routes have other nonstop carriers on them.

All in all, the survey is pretty clearly an algorithm that's trying to isolate which options a flyer cares about/would consider paying more for, and if so, what levels/how much more (and that's likely how the results would be presented back to whoever at DL commissioned it)... but whoever coded the survey isn't someone who actually flies or understands the purchasing patterns of frequent flyers.
