Interesting. I got the leisure version of the survey. I guess because I have not flown DL internationally since 2016. All my international travel is on Singapore, ANA, JAL and BA.

It started with a section of my flight activities (all US airlines) last year and my opinion of DL in general. In addition to 14 hypotheticals of leisure flights of 1-3 hours involving various bundles. Took over 15 minutes to finish the survey.

A few things I noticed. It seems to me they are definitely looking at possible ways to bring back cancellation fees. Also trial ballooning fees for phone interactions for help. Right, I am going to pay $25 for the privilege of getting an agent who might not have a clue..There were also options (including Comfort and various FC fares) that did not allow Same Day Changes at all. I wonder if that too might be on the chopping block. Seats assignments other than those assigned on the day of departure (including some FC bundles) for higher pricing. A couple of options for FC that included being in "the middle" or back of FC for seat assignments.

For every page of hypotheticals, I chose the options for 1) MC fare. 2) No I would not fly the fare I selected. And 3) that I would check with other airlines. By the time I got to the opinion part, I was prompted to explain why I rated DL poor. And asked what my rating was post hypotheticals. I responded with Slightly worse. And gave them a detailed response of what I think of the current services (inflight, GA and phone). A shame I won't get a response. But hopefully they are getting lots of similar feedback.
