It is a question that pops up ever so often in various Japan related hotel threads.

There are not that many hotel rooms in Japan that does accommodate three adults in the same room, and for the international chain hotels it is typically more premium rooms or suites, or the more family oriented destination areas like close to Tokyo Disney, beach or onsen resorts. Local chains probably have more family style rooms, often in a Japanese style with tatami sleeping areas. It will be tricky in central Tokyo and on points.

I would advice against booking two adults and showing up three and assume they will accommodate you.
