No. To my knowledge, only American Express and Capital One have opted out. Bank of America, Chase, Citi*, US Bank, and various smaller issuers include restaurants. (*Citi does not currently issue cards which include Priority Pass, but existing Prestige card accounts retain Priority Pass.)
Chase is the big player there. If I were in their marketing department, I'd be hell-bent on trumpeting this substantial benefit that Chase's main competitor in this market (Amex Plat) does not provide.
