The Trenord.it website works fine for me, but you can also buy the MXP Express tickets on the Trenitalia App once you land and are waiting for your bags. Remember you need to 'check in' for your train to Milan. Oh, and select a MXP express service going to Centrale (or Garibaldi), the Cadorna one won't be of great use if you need to catch a train for Bologna.
Thanks for the tips. Oddly enough, when I signed into the VPN on my work laptop, the Trenord website loads great. But still will not on my iPhone or home iMac. So I just downloaded the app and that works great too.

#4 2967 / 18:13 - MALPENSA AEROPORTO T1 --> 19:07 - MILANO CENTRALE (1 hour 48 minutes to catch MXP Express)

I would say #1 is next to impossible, #2 everything needs to go perfectly, #3 reasonable with no problems, #4 too much time (unless you have a big problem).
We had visitors from US do a similar route last summer (they were taking the Frecciarossa to Parma where we picked them up). They did the equivalent of #3.
Bottom line, I would agree with 13901.
This timetable helps tremendously, and was the first thing I was looking for on the Trenord website. Totally agree with your analysis. #2 could work if our flight in from Lisbon arrived early, bags came out on time and the baby didn't need a diaper change! So we should surely expect #3 to be most likely. Realized too that the Freccia Family tickets allow for time changes, so if we do get to Centrale early (or late!) we can just move to the appropriate train.
