You don't add it. It is an automated process given you are using the same account info when you applied for the Blue Plus. I logged in when I applied and it was automatic added to my Amex card rolodex on Day 1 and the MR account was fully linked on Day 2.
I always had to manually add my cards to the online account after I received the physical cards. It was never automatic.
