In the same way that 15-20 years ago I bought an "extra checked bag" for Air Asia, yet couldn't check it in because I hadn't purchased "extra weight". You make assumptions or you make mistakes, especially if you are unfamiliar with an airline's policies.
Presumably the woman bought a checked bag ages ago then decided that she didn't need to bring so much stuff, and made the basic, incorrect, assumption that her bag would be allowed onto the airplane either way. Her other mistake - which is how I found out - is that she was asking the Group 3 queue whether we had bought overhead bin space that we weren't using. (if we had, we wouldn't be in Group 3
