You do not need an approval from us for carrying personal medications if you are only transiting through Singapore without clearing the Singapore immigration. Do note that prohibited substances such as chewing gums, cannabis and product containing cannabis extracts, are NOT allowedto be brought with you even if you are not clearing immigration.

There is an online tool where you can enter the active ingredients. Plain nicotine patches - it appears you can bring in three months supply, so if that is right, transit should be fine.
