Based on THIS report I expect you will be billed $195 for the additional user.

I believe American Express will issue the card without SSN, but sometime thereafter they will contact you to provide one, and if you do not they will close it. If you explain that the cardholder does not have an SSN they will tell you the acceptable alternatives.
