So, I've deciphered some of what's going on, although I don't have a good
explanation for it. ITA Matrix is using a different fare than ExpertFlyer is presenting. The fare code is the same, but the rules are different. The one used by ITA changes the allowed routings -- eliminating AS, for example -- and changes the transfer rules, which is what allows the transfer in SIN in the first place.
w.r.t. trying to fly KUL-SIN-TYO-SFO, anyway, I was able to get the fare to come through. However, it's adding an extra ~$300 in YQ. Since I've never quite worked out where I can look in the fare rules to see what the YQ is, I basically had to do a side-by-side comparison of
f SIN f+ vs.
f SIN o:nh+ to see what changed.
If you look at the fare in ITA -- again, not in EF -- you'll also find surcharges written into the fare as follows (again, this is from the SFO fare):

So this probably explains the problems people have been getting trying to price HKG.
