yes, i have. They made copies of my cards and receipt and said they submitted a request through a some kind of portal in their computer to corporate or BHN to investigate this. It seems this is the part that takes a few weeks to resolve and get refund. IME, the store GMs tend to be helpful. However, my friend dealt with a GM who kept telling him to deal directly with BHN when BHN says it's OD who should be looking into this.
The issue is with OD, not with BHN.
It is the responsibility of the store GM. When I had issue years ago, the GM first contacted Corporate where he was given a special tel number to talk to, presumably is the corporate fraud dept. They requested the photocopy of the package and the cards. It took about 3 weeks to get an approval from that dept on resolution. The GM gave me replacement cards already activated with receipt for me to pick them up at the store.
For your friend, I suggest if he could not get any progress, contact the corporate directly,
