Lounge passes can only be used in DOH. As you are Platinum it will be for the Platinum Lounge.
Yes, you will gave to use one pass for the extra guest.
At the outstations you will be allowed lounge access plus two guests only.
Do note however that in BKK you will be granted access to the Miracle Lounge and not the QR Premium Lounge.
