But the rep made the LAX to HND in premium economy so it was pricing out too high. For this one in particular, putting them into ITA Matrix, I could found fares by splitting the following way - 3 price sets with 4 fares involved, you were lucky when the agent approve this set if the 3 fare limit is there.

MIA-LAX-HND-SGN (2 fares in single quote)
Furthermore, they seem to have restricted the itinerary so that RTW with deliberately multiple stopover is almost impossible.
Had you removed CMN stops and go direct to MIA I guess the success rate would be higher as it look more direct from APAC to USA; it may help further by removing the DOH stopover as well...
My US-bound multi carrier reward from HKG with outbound via DOH and inbound via TYO and KUL was approved only on condition that there are no further direct routing from HK to US and JP to HK at the time of booking, and that the DOH and KUL stop MUST be transit only - such notes were left explicitly in the ticket notes.
