If you don't use it, then at some point in the future you will need to regain Diamond the normal way or via a status match. A year or two after that - if and when your Diamond status is going to be lost - you could apply for the Diamond extension then. Obviously we talking about long-term projections here and program rules can change - but if you think that the late 2020s might see more travel then you should hold off.

I'm in the same boat. I know that I won't stay hardly at all at Hilton in 2024 - Hyatt, IHG and Marriott all treat me better and give me a better earn/burn return... So I'm probably going to pass on the extension and see what happens later in the decade...
