I have had your very issue before. More often then not, it seems that an added Rail&Fly segment will not properly show up as part of such a booking.
My solution was to have the problematic Rail&Fly segment cancelled and refunded, and have the agent create a new, completely separate booking for the Rail&Fly, and ticket as such. This worked flawlessly for me, but I did have to instruct the agent quite specifically on what to do—if your agent is unwilling consider HUCA.

As I understood it, they have to refund the rail and fly part, and leave my lifemiles award as it was orginally. Then create a new booking and ticket -- this is the part that confuses me the most-- (from where to where? could you give more details) and then onto this ticket add the rail and fly?
Would going to the FRA train station work?
