Pics over at pprune show that the aft hinge collar/bracket came loose from the plug assembly and stayed with the hinge. From the position near the end of the post it looks like the retaining bolt was missing or gave out. Given that the hinge doesn’t look bent out of place, I’d guess missing/fell out before the plug started to deploy.
