I only went back to 1970, but I can’t readily find any examples of human-failure/airframe failure sudden/explosive decompression incidents that took place after the first hour of flight other than Delta 172, an L-1011 that had an aft pressure bulkhead failure two hours in to a flight from LAX to HNL. They returned to LAX at FL14000 with no passengers requiring hospitalization.
