i like the "building door that's hinged to open outward but doesn't have a handle"

it seems to me that the plug should PUSH against the frame when pressurized rather that PULL the frame when pressurized

the pics that show the alignment plugs are "clean" tells me that plug components are what failed

hopefully the door is found today intact and "clean", the plug mounts and "bolt holes" are also "clean", and then it's down to bad bolts (materials) or human error (bolts weren't there)
