My recent flight from KUL was switched from Qsuite to old 2-2-2 just 1 day before departure (and seems to have happened in the previous week too based on flightradar24), so I would think you should always assume you're flying the old 2-2-2 on that route unless you take the 787 flight departing later on the same day (or fly to SIN for almost guaranteed Qsuite).
I think they might keep flying these old 777 till they replace them with newer planes because they are mostly over 10 years old now and QR probably doesn't plan on flying them for another 5-10 years to make refitting the seats worthwhile, all while they're also lacking planes to expand their network.
