In some cases the ticket number doesn't change, for example if you just change the date, the existing ticket is 'stickered' and not reissued. The policy for travel agents is detailled here

A reissued ticket will have a new ticket number. Seeing the same ticket number should have raised a red flag, but how can the average passenger know?
It didn't. I only noticed this after I had missed my flight and was investigating.
This is an interesting discussion, and I had a couple more questions -
- Does Art. 7 EU Regulation 261/2004 apply in this case (putting aside the fact that it was CapitalOne's fault in this case)? I had tried to file a claim for Art. 7 EU Regulation 261/2004 using lufthansa support (compensation-in-the-event-of-flight-irregularities). However, I got a response stating that this regulation does not apply because my flight was from BLR to SFO (not to/from an EU country). This was strange considering that Frankfurt was the transit country.
- Monetarily, I was only out ~20$ totally. However, it was only because I had spent an entire day following up with both customer agents. So I'm wondering if it's even worth pursuing this with either parties.
