THB 500 seems to be the fixed rate they ask if not going by meter. Sounds about right.
The only scenario I can think of where you might come out ahead is when you agreed on a fixed rate but you're stuck in traffic jam for hours
I believe if you go by meter, it charges you THB 2 for every minute a vehicle isn't moving faster than 6kph(?)...

With regards to the tolls I think it depends on the driver, sometimes I have been asked to pay them on top of the agreed rate, very rarely was I not asked to contribute my share.

But yes, bottom line – Grab is the way to go. You know what to expect 99% of the time, no guessing games.
I mean on the 500 I have always insisted on the toll being paid by them before I book it or get it booked. Yes very rarely I have been in the situation of a fixed rate and then stuck in traffic and then come out on top in that sense. Last week I told him not to take the toll way but somehow he took and he did not charge me for the toll and it was 80 baht. It definitely cut short my time a lot and I did give him 35 though he did not ask for it. This was a local ride and not to the airport.
