I just booked a trip, and also got "404 page not found" instead of booking confirmation page. There was no email confirmation either, Avios are still in my account. However, I see it under "Upcoming trips" and even have all the details.
Yes, I think I saw all my attempts in Upcoming Trips, but they automatically got cancelled after a time as the payment never worked.
