One thing to keep in mind is that the center seats are accessed from different aisles, so for example if you need to get something from the overhead bin/locker for your grandmother you'd have to walk all the way around to the other aisle, or store it in the bin on your side instead. You will be able to reach over somewhat awkwardly to help with things like headphones or tray tables, but it won't be as easy as being on the same aisle. However, talking is much easier between E/F than between A/E.
You don't say who the third member of your party is, but one option is that your grandmother takes the E seat, whoever is in A helps with physical tasks, and whoever is in F is the social person (and maybe the monitor if that's required). Another option would be to go with forward adjacency, eg 4A/5A or 4E/5E, though it's harder to talk to each other this way.
