Code share doesn't matter directly, but it can complicate things. What matters is the operating carrier and their fare class. For a codeshare, it can be difficult / impossible to determine the fare class
according to the operating carrier.

Look at footnote #1, and then compare to your receipt.

Definitely not true. It also needs to be an eligible operating carrier and fare class. You can book Cathay Pacific flights on a 016 ticket; they don't earn any MileagePlus miles, though.
