Sounds like the re-issue at EWR was the cause of the problem. Upgrades and re-issues don’t go together. Sounds like an error that your Email and receipt for the SAUA fee might be able to solve. Did ET get back to you after you sent them that reply claiming otherwise? UA should be able to help with that, if there is a continuing issue. What can sometimes happen with re-issues is that UGs get reversed; did any of the miles get returned? Does UA show the UG as cancelled.
i've had SAUA re-issued after confirmation, and never got the miles back until much later when I requested it via support. I doubt OP got the miles back, regardless of if s/he lost the upgrade status on re-issue
