No to all the above. If you buy an extra you’re not changing the form of payment, you’re adding a new one which will be assigned to the new document, not the old one. If you eventually refund this you should end up with two refunds, one to each card for each element.
The only current solution is to call. I’d like to think that in a future order management system Amadeus will plan for this and allow a refund to be fudged the way it can offline, but then Amadeus are not exactly known for common sense. The banking industry has accidentally come up with a half-solution themselves in that they’ve started issuing cards with much longer validity periods than they used to - entirely out of cost-saving in not having to print and post as many, but meaning it’s only an issue once every 5-7 years, rather than every 2-3.
