The only place I've found that did it properly was Ray's Tavern in Green River, Utah. I was sure I had taken a picture, but I can't find it. This is from the Google photos of the place and was taken by The Swell Utah in 2021.

Sadly, more recent photos suggest that as of a year ago they list only the cash prices with a note that "There is an extra charge if you pay with a card."
