Since your card will not be expired on your travel date there will be (should be) no problems with airline agents. However, even if the rules are clear, the airlines are very risk averse in these situations and might (and are allowed!) refuse to believe their own eyes reading the US government rules and still not allow you to board. In which case you will need an I-131A form.

My suggestion for you: while at the airport on arrival to India - visit the check-in desk and ask them to verify your documents are acceptable for your departure. Get names and titles (business card!) of whomever assures you everything is fine. If they say your documents are not fine, then you now have time to get the I131A....
