I would expect American Express to bill the $195 fee for each Additional Cardholder on the anniversary of the card opening. I would expect a pro-rated refund of the most recent $175 fee, but in the case where there are multiple Additional Cards with different anniversaries I would expect the refund when the last of the Additional cards renewed.
